Due to the capacity of the conference room, the maximum attendance limit has been set at 150 people. Could you please submit the following Registration form as soon as possible. Registration will close 31st January 2014.

All attendees will be invited to a Banquet (cost is included in registration fee). Could you please mark the Banquet option button in the form bellow. In addition, we are planning to take attendee on an excursion. We are now planning to go Japanese Red Cross Ishinomaki Hospital in Ishinomaki - one of the major sites of the last earthquake. We will inform you of those plans from the home page.

Please note, the poster presenters (except invited speaker) MUST submit BOTH the Registration form and the Abstract submitting form.

For invited speaker, please submit ONLY your abstracts through following Abstract submitting form by the end of deadline.

Registration fee is 5,000 Japanese Yen including abstract book, 2-days lunch and Banquet. In addition, we will ask to pay couple of thousand yen the persons who will go excursion.
